Acute myocarditis is an inflammatory disease of the myocardium that may clinically mimic acute coronary syndrome. A 28-year-old man presented with chest pain, dyspnoea, fever and fatigue following acute otitis media. Elevated inflammatory markers and troponin prompted coronary angiography, which demonstrated normal coronary arteries. Cardiac magnetic resonance imaging revealed myocardial oedema and subepicardial late gadolinium enhancement consistent with acute myocarditis. The patient responded well to medical therapy and was discharged in stable condition.
